Reveal Findings

FL – Anterior urethral diverticulum with late micturition obstruction

Reveal Discussion

Urethral diverticula may be congenital (hypospadias, dilated Cowper’s duct/gland) or acquired (trauma, surgery, catheterization). Patients with anterior urethral diverticulum present with ventral penile swelling, incontinence, poor urinary stream and/or infection. Anterior diverticula may be associated with or without an anterior urethral valve. Without a valve, progressive distention of the diverticulum leads to valve-like obstruction of the distal urethra. With valve, obstruction by valve distends the diverticulum, undermining and further compressing the anterior urethra. Anterior urethral diverticula are treated by endoscopic deroofing, open diverticulotomy or open urethroplasty.
