Reveal Findings

MR – Sagittal T2, STIR and post-gadolinium T1 images of the thoracic spine shows multilevel extradural compression of the thecal sac and cord edema but normal vertebrae.

Reveal Discussion

Extranodal presentation can be found in 25–30% of patients with lymphoma, although epidural presentation is relatively rare. The extranodal lymphoma can be found at any age, however it is most common in late adulthood. Intraspinal lymphoma spreads from adjacent nodes through the intervertebral foramina or may reach the epidural space from the vertebral body; months may pass before the development of pain or spinal cord compression. Lymphoma is most commonly present in the mid-thoracic and lower lumbar segments.
