Renal function subsequently normalized. Followup renal ultrasound 1 week later also showed normalization.

Reveal Findings

US – Striated hyperechogenicity surrounding the medullary pyramids.

Reveal Discussion

The etiology of the striated hyperechogenicity surrounding the medullary pyramids in this case is uncertain but may be related to 1) precipitation of crystalloid, calcium or protein, 2) vascular congestion and or hyperemia, or 3) tubular stasis and or ectasia initiated by the patient’s sepsis.
