Reveal Findings

CT – Axial and sagittal and 3D reformatted images show a circumscribed fluid attenuation mass in the anterior fontanelle with smooth bony remodeling.

MR – Sagittal T1, coronal T2 and sagittal post-gadolinium T1 images show non-enhancing, minimally heterogenous but predominantly T1 hypo intense and uniformly T2 hyper intense extradural intrasutural lesion.

Reveal Discussion

Dermoid of the anterior fontanelle is the most common site of the skull and face after those of the eyebrow. They are twice as common in females than males. Dermoids may be noted at birth or in the first months of life. They enlarge slowly as a result of the accumulation of secretions within them. They are nontender, fluctuant, and slightly mobile. The differential diagnosis of a mass lesion at the anterior fontanelle includes hemangioma, lipoma, epidermoid cyst, sinus pericranii, meningocele and encephalocele.
