Elevated vanillylmandelic acid (VMA).

Reveal Findings

US – Hydrocele with hypervascular mass adjacent to the normal testicle.

Gross Pathology – The tumor originates from the epididymus. The cut surface of the tumor is white and fibrous with a punctate central dark spot.

Reveal Discussion

Melanotic neuroectodermal tumor of infancy (MNTI) is a rare tumor. More than 90% involve the head and heck (most often the maxilla) but skin, mediastinum, long bones, uterus, ovary and epididymus have been reported. MNTI are almost always solitary with an average age of onset between 1 and 6 months. While MNTI grow very rapidly, the clinical course is benign with only 15% incidence of local recurrence.
