Reveal Findings

CT – The sagittal reformat shows the posterior superior corner of C7 vertebral body is displaced into the spinal canal. The coronal reformat shows the characteristic vertical fracture line. The axial images show additional fracturing of the posterior elements.

MR – Sagittal T1, T2 and IR images show defect in the superior C7 endplate and minimal cord edema.

Reveal Discussion

Burst fractures occur during axial loading with the spine in neutral (no flexion or extension). Compressive force is transmitted through the disc and dispersed through the endplate of the vertebral body leading to centrifugal displacement of vertebral body fragments.
