Gardner Fibroma

Findings

Lobular mixed well and ill defined very hypointense T1 and T2 non ehancing lesion in the soft tissues of the medial knee, inseparable from the medial capsule and vastus medialis.

Discussion

Gardner fibroma (GAF) is a benign soft tissue lesion with thick, haphazard collagen and bland fibroblasts that entrap adjacent tissue.

Uncommon; extra abdominal type of fibroma more commonly affects infants, children and teenagers. Although most cases are sporadic, familial-related cases occur, especially in patients with Gardner syndrome.

May be initial diagnostic clue to Gardner syndrome and APC mutations.

These lesions have a heterogeneous MR imaging appearance, mostly with intermediate or low signal intensity on T1-weighted images and with high signal intensity mixed with bands of low signal intensity on T2-weighted images. The areas of high signal intensity on T2-weighted images usually show avid enhancement with gadolinium. The tumor usually has both well-defined and ill-defined margins.
