6 week old female with left inguinal mass.

Diagnosis

Ovary – Left Inguinal Hernia

Findings

US – Ovoid mixed echogenic mass in left groin with vascular pedicle traversing the inguinal canal.

Surgery – Left fallopian tube and ovary in left groin and inguinal canal.

Discussion

Most congenital inguinal hernias involve bowel, omentum/mesentery or encysted fluid. While the male gubernaculum is instrumental in drawing the gonad through the inguinal canal, the female gubernaculum (round ligament) connects the uterus to the labium giving rise to uterine anteversion and restricts inguinal herniation of the ovary. Consequently ovarian inguinal herniation is very rare.
