Diagnosis

Splenic Epithelial Cyst

Clinical Notes

Hemorrhagic intrasplenic cyst at surgery; epithelial lining confirmed on pathologic evaluation.

Findings

US – Intrasplenic cyst with uniform thin wall and low-level internal echoes.

MR – T1 and T2 hyperintense intrasplenic collection with low signal rim, consisent with proteinaceous contents and hemosiderin respectively.

Discussion

True cysts of the spleen (also called epidermoid cysts) are rare. These lesions are defined by the presence of epithelial lining. Pathogenesis is thought to the result of infolding of peritoneal mesothelium. Low level internal echoes are the result of cellular debris, proteinaceous secretions and hemorrhage. The cyst wall should be smooth and uniformly thin. CT or MR may be necessary to confirm the cystic nature and further characterize.
