History

3 year old with herniation of cystic mass through a healed inscision over the left hip.

Diagnosis

Hip Joint Synovial Cyst

Clinical Notes

History of open reduction and tenotomy for hip dysplasia.

Inflamed synovium extending into the subcutaneous soft tissues, arising from the anterior hip joint capsule near a suture granuloma found at surgery.

Findings

US – Contained complex subcutaneous fluid collection funneling toward the hip joint.

Discussion

Herniations of synovium through the joint capsule (i.e., synovial cysts) may be unilocular or multilocular. These herniations are prone to dissection into adjacent soft tissues, rupture and infection and can compress structures. At least 15 synovial bursae are found about the hip. Any condition resulting in a joint effusion and increased intraarticular pressure predisposes to formation of synovial cysts.
