Neurofibromatosis with pathologic fracture

Findings

CR – Lateral view of the leg demonstrates multilocular cystic lucency with segmental sclerosis in the anterior proximal tibial diaphysis with associated apex anterior tibial bowing. Note the transverse fracture through the anterior aspect of the tibia.

Discussion

The cystic lucencies in this case likely represent a combination of non-ossifying fibroma, fibrocortical defects and mesodermal dysplasia. Osseous dysplasia weakens the bone and consequently predisposes to fracture. Nonunion and pseudoarthrosis occur in as many as 13% of these pathologic fractures.
