History

7 month old female with non-pulsatile mass in the anterior fontanelle.

Diagnosis

Dermoid Cyst

Clinical Notes

Pathology – Cystic mass containing grumous gray-white material with pale hair.

Findings

US – Mixed echogenic subcutaneous hypovascular mass in the anterior fontanelle external to dural and sinus.

MR – T1 hypointense and T2 hyperintense scalp mass.

Discussion

Dermoids are benign lesions containing hair, skin appendages, fat, sebaceous glands, and keratin (NOT cholesterol). Dermoids are usually midline or near the midline. Common sites include the superolateral orbit, scalp, brain, nose, neck, mediastinum and ovaries.

Dermoid is the most common scalp mass; differential diagnosis includes cephalohematoma, Langerhans cell histocytosis, angiolymphatic malformation, neurofibroma, chloroma, meningo/encephaloceles.

On MRI dermoids exhibit mixed T1 signal, predominantly high T2 signal and variable enhancement. On sonography dermoids are cystic with variable echogenicity.
